| Use this form to sign up for Auto Pay using ACH (Automated Clearing House)/electronic funds transfer (EFT). By filling out the ACH form, LCM will automatically debit your bank account after the 4th of each month for the amount of your current regular assessment amount and any other regularly occurring assessments.
